      Oak Ridge, Tennessee       Type B              work location.

    OR/BNFL - Oak Ridge          03/26/99   Injury   Employee received multiple head and neck injuries when he was struck by
      Oak Ridge, Tennessee       Type B              a section of chain link fence which was catapulted onto him during removal.

    AL/Albuquerque Transp. Div.  04/18/99  Fatality  A security guard collapsed and died following a mile run qualification  
      Albuquerque, New Mexico    Type A              and 10 minute cool-down.  Cause unknown.

    PA/Western Area Power Admin  05/19/99   Injury   Two electricians received electric burns when an energized system arced 
      Hayden, Colorado           Type B              during nearby maintenance tasks.

    OR/BNFL - Oak Ridge          06/24/99 $1,650,000 A localized high strength wind caused damage to the K-33 building 
      Oak Ridge, Tennessee       Type B

     Note:  DOE Order 225.1, Accident Investigation, revised the thresholds for reporting Type A and B incidents.  However, contractual 
            obligations for compliance with this order are varied, hence the reporting of such incidents is not consistent across the 
            DOE complex.
