OR/Lockheed Martin Envir. 03/20/99 Injury Employee passed out injuring face and jaw while walking up a hill to his
Oak Ridge, Tennessee Type B work location.
OR/BNFL - Oak Ridge 03/26/99 Injury Employee received multiple head and neck injuries when he was struck by
Oak Ridge, Tennessee Type B a section of chain link fence which was catapulted onto him during removal.
PA/Western Area Power Admin 05/19/99 Injury Two electricians received electric burns when an energized system arced
Hayden, Colorado Type B during nearby maintenance tasks.
