      Los Alamos, New Mexico       Type A             causing extensive fire damage to buildings, vehicles, and equipment.

    OR/WackenhutServices - OR    07/07/2000 Fatality  Two employees were killed and another injured when their vehicle was struck 
      Oak Ridge, Tennessee         Type A             by a runaway utility trailer.

    SPR/DynMcDermott Petroleum   11/14/2000 Fatality  Employee succumbed to injuries received when his vehicle was struck by a another
      Roanoke, Louisiana           Type A             vehicle which had come loose from a tow truck.

     Note:  DOE Order 225.1, Accident Investigation, revised the thresholds for reporting Type A and B incidents.  However, contractual 
            obligations for compliance with this order are varied, hence the reporting of such incidents is not consistent across the 
            DOE complex.
