Safety Management Through Analysis DOE Operating Experience
Lessons Learned

Analysis of events for lessons learned is a basic tenet of safety management. Various analytical approaches can identify, in events and families of events, features such as risk of injury or fatality, safety significance, number of barriers to failure, comparison to industry averages, and cost-benefit of corrective actions.

Like many other industrial organizations, the Department of Energy requires lessons-learned identification, evaluation, sharing, and incorporation into projects, programs, or operations. This reflects the need to effectively communicate acquired knowledge and to ensure that beneficial information is factored into planning, work processes, and activities.
