Update on Electric Fan Safety Alert
from the General Services Administration


DISCUSSION


Safety Note DOE/EH-0160; Issue No. 90-5; September 1990; was issued to alert DOE facilities of an occupational safety hazard caused by a design deficiency in 16-inch oscillating table-top fans. The fans; manufactured by Lasko Metal Products; Inc.; of Fort Worth; Texas; may cause sparking and electrical shock. Following is information just received from the Government Services Administration (GSA) regarding the fans.


ACTION NEEDED


The defect in the Lasko fans can be corrected by replacing the existing metal base plate with one of new design. In order to receive the necessary material and instructions to make this correction; which takes only a few minutes; a Standard Form 368 must be completed by your property management office. GSA's Safety Alert No. FQA-1/90 must be cited and the following information must be provided.

  7. MANUFACTURE DATE (MONTH AND YEAR SHOWN ON THE UNDERWRITERS LABORATORY (UL) LABEL); EXAMPLE: 7/88

  8. CONTRACT NUMBER (SHOWN ON THE SAME LABEL); EXAMPLE: GS-OOF-94123

  9. AGENCY REQUISITION NUMBER (IF AVAILABLE)

  10. NAME AND TELEPHONE NUMBER OF A PERSON TO CONTACT
