DOE/EH-0160 --- Issue No. 90-05 --- 09/90
Electric Fan Safety Alert from the General Services Administration
DISCUSSION
The General Services Administration (GSA) procured 16-inch oscillating table-
top fans for the Department of Energy (DOE). These fans were manufactured by
Lasko Metal Products, Inc., of Fort Worth, Texas. GSA has advised DOE that
these fans pose an occupational safety hazard. The problem is attributed to a
design deficiency that may result in a loss of insulation and cause sparking
and electrical shock. GSA also advises that 12-inch fans manufactured by Lasko
do not have this design deficiency.
ACTION NEEDED
Lasko 16-inch fans should be taken out of service and stored until further
instructions concerning their disposition are issued by GSA. In addition,
these fans should be tagged with a warning (e.g., "shock hazard") while in
storage. Please contact Jim Michalakis, GSA (FTS 557-7997), or Eleanor
Crampton, DOE/EH (FTS 233-3732), if you have any questions concerning this
matter.
This Safety Note is one in a series of publications issued by EH to share occupational
safety and health information throughout the DOE complex. For more information, contact
Eleanor Crampton, Safety Performance Indicator Division, Office on Environment, Safety,
and Health, U.S. Department of Energy, Washington, DC 20585, (301)903-3732.
