GROUP 1 - FACILITY CONDITION

G. Oversight Activities

Unusual Occurrence

Any internal/external oversight activity discovering unsatisfactory operation, testing, maintenance, or modification of any Safety Class SSC that is required to be operable or in operation.

Off-Normal

Any internal/external oversight activity discovering unsatisfactory operation, testing, maintenance, or modification of any Safety Significant SSC required to be operable or in operation.



INTERPRETATION
Issue:

Group 1.G criteria generally would always be reportable under Group 1.E. How should this criterion be applied in relation to Group 1.E and 1.F?

Intent:

Group 1.G should be used when an internal/external oversight organization identifies the reportable event or condition.

Additional Information:

It is a significant event when oversight organizations find reportable conditions or events before the responsible operating organization. The facility's quality assurance organization should be considered part of the responsible operating organization, but a corporate or DOE assessment group should be considered oversight. It should also be noted that other criteria would always be met requiring reportability. Group 1.G is an additional code to further identify how the occurrence was found.
