Potential Concerns/Issues Off-Normal (1) Any event resulting in the initiation of a Type A or B investigation as categorized by DOE O 225.1, ACCIDENT INVESTIGATIONS. INTERPRETATION Issue: Is the intent of the Off-Normal criteria to identify two Natures of Occurrence for any event that results in the initiation of a DOE Type A or B investigation, since the event would also meet other reporting criteria? Intent: Yes, this nature of occurrence should also be selected so that a link can be made to occurrences that result in the initiation of a Type A or B investigation.
