Approved 10/27/97 REQUIREMENT (4) Facility Manager/Designee. Enter the name, title, and telephone number of the Facility Manager or designee who has direct line responsibility for operation of the facility. If ORPS is being used, enter the name, title, and telephone number of the responsible Facility Manager or designee who approved this report, either by personally transmitting the electronic report or by signing the hard copy report. NOTE: ORPS will not automatically enter the name of the Facility Manager in this field. INTERPRETATION Some facilities have not properly identified the Facility Manager/Designee. Instead, in some cases, the OR Investigators have filled in their names in this area, as well as having their names in the Originator/Transmitter Block. Intent: To identify, by name and telephone number, the person who has occurrence reporting responsibility for the facility. The concern is that the name and phone number identified on the reports is not the person responsible for facility operations, including the authority to direct physical changes to the facility. Additional Information: For each report associated with an occurrence, the name entered in Field 4 (Facility Manager/Designee) should be the name of the Facility Manager/Designee who approved that particular report, either by personally transmitting the electronic report to ORPS or by personally signing the hard copy report, which should be retained for inspection. For each subsequent report associated with the occurrence, Field 4 is always blank and must be filled in with the responsible Facility Manager/Designee who signed or transmitted that particular report. In addition, on Final Reports, the name in Field 4 will automatically be entered in Field 36. |
