April 24-27, 1995 Charlotte, North Carolina
The TRADE Occurrence Reporting Special Interest Group (OR SIG) Spring Meeting was held April 24-27, 1995 in Charlotte, North Carolina, with approximately 150 OR SIG members in attendance. OR SIG Steering Committee Chair, Dan Connell, Westinghouse Hanford Company, formally opened the meeting and introduced Andy Marchese, DOE/EH-33, who spoke about the importance of using risk analysis for prioritizing events by their safety and risk significance and cost/benefit. OR SIG Advisor, Eugenia Boyle, DOE/EH-33, spoke about the status of DOE Order 261, "Occurrence Reporting and Processing of Operations Information" (DRAFT), which will replace 5000.3B (and draft 5000.3C), and the changes associated with this new order. The reporting criteria, which was previously in Attachment 1, will now be included in a DOE Standard, along with the other attachments. Since comment consolidation for the draft 5000.3C was complete, Order 261 will not be issued for comment. It will be issued as a final order. Updates on OR SIG task team activities were given by the Policy, Resource Guide, Users' Group and Training Task Team Leads.
Three (3) Steering Committee members were elected during the OR SIG Spring Meeting: Mark Hartman, DOE/EM-62; Rick Goodell, Los Alamos National Laboratory (LANL); and Kim Wilson, West Valley Nuclear Services (WVNS).
Certificates of appreciation were given to two outgoing OR SIG Steering Committee members: Susan Jennings, Martin Marietta Energy Systems, Inc., and Frederick (Tom) Daniels, DOE/RL, and also to Kim Wilson, WVNS, for her previous term.
Hot topic sessions were conducted on the following subjects: Lessons Learned, Computer-Based Training, Generic Reporting, and Noncompliance Reporting of "Violations" at Nuclear Facilities. In addition, presentations were given on The Use of Multimedia in Occurrence Reporting by Bill McQuiston, DOE/ID, and The Noncompliance Tracking System by Sharon Singh, DOE/EH-13.
The OR SIG Spring Workshop was highlighted by the presentation of three training courses - Occurrence Report Writing taught by Dennis Lowry, DynMcDermott Petroleum Operations Company, Inc.; Reportable Occurrence Investigative Techniques, taught by Al Elliott, LANL; and Understanding Cause Codes, taught by Kim Wilson, WVNS, and Rick Goodell, LANL. The materials for these courses were developed by members of the OR SIG Training Task Team and other OR SIG members.
