OPENING SESSION
The Occurrence Reporting Special Interest Group (OR SIG) held its fourth annual Spring
Meeting in conjunction with the Occurrence Reporting and Processing System (ORPS)
Workshop, March 11-14, 1996, in Idaho Falls, Idaho, with 136 participants.
Opening remarks and welcomes were given by Dan Connell, OR SIG Chair, Walt Sato, DOE-ID,
and Greg Baker, Lockheed Martin Idaho Technologies. Lou Wagner, General Director of Union
Pacific Railroad, was the keynote speaker and talked about Union Pacific's accident
reporting system. Union Pacific is the second largest railroad in the United States with
35,000 employees, 3,922 locomotives, and 97,600 cars owned. They spent approximately
$970,000,000 in 1995 on their capital improvement program.
Tom Rollow, Director, Office of Operating Experience Analysis and Feedback (EH-33), talked
about what's happening in EH-33, EH-33 products and databases, and the importance of ORPS
users' and the OR SIG in helping EH-33 achieve its' mission, improve efficiency and save
money.
In addition, a discussion of ORPS Enhancements was presented by Bob Lyon, LMIT, and an
ORPS Graphical User Interface (GUI) demonstration was presented by Hortense Nelson,
Scientech, Inc.

The OR SIG and ORPS Home Pages were demonstrated to attendees by Bill McQuiston, DOE/ID.

OR CBT COURSE
Helene Thiss, FERMCO, demonstrated the Occurrence Reporting (OR) Computer-Based Training
(CBT) Course that the OR SIG Training Task Team is developing with the assistance of the
Special Interest Group for Advanced Training Technologies (SIG ATT).
The CBT course consists of three modules. The first module is a tutorial which includes
the following components: an introduction to ORPS, responsibilities, discovery,
categorization, notification (classified/UCNI Reports, general items: corrective actions,
lessons learned, trending and analysis, multi-contractor events), and initial
investigations. Module Two consists of three possible scenarios (a vehicle accident, fire,
or asbestos). Students will choose one of these scenarios to practice going through the
ORPS process and writing an ORPS report. They will then choose a second one to go through
and be evaluated. Therefore, they will only see two of the three scenarios per training
session. Module Three is the ORPS Software Simulation. According to the student's ORPS
User Level, they will be trained on one of the following: General Users:
Report item searching, identifying the status of reports, identifying corrective actions,
identifying components of reports, creating/using a search profile, finding report
information, looking up an authority file, and using search logic; Facility
Representatives: Creating/ submitting comments, identifying reports needed
review, and approving/ rejecting reports; and Facility Managers:
Identifying reports which require review.
HOT TOPIC AND DISCUSSION SESSIONS
Hot topic sessions were conducted on the following subjects: Computer-Based Training;
Re-Engineering DOE Reporting Systems; Quality Tools for Problem Solving/Roadmapping the
Future; and Quality in Occurrence Reporting.
In addition, discussion sessions were held on DOE Order 232.1 Interpretation and
Consistency in Reporting and Emergency Preparedness vs. Occurrence Reporting.
OR SIG STEERING COMMITTEE ELECTION
The OR SIG extends a welcome to the new steering committee members who were elected during
the 1996 Spring Meeting: Dan Connell, WHC, who was also re-elected as OR SIG Chair; Lorna
Dunn, RFETS, DynCorp of Colorado; Al Elliott, Los Alamos National Laboratory; Cynthia
Eubanks, LMES; and Tim Quinn, Bechtel-Hanford.
TASK TEAM UPDATES
Lessons Learned Task Team - Dick Trevillian delivered a
presentation on a new EH-33 product, Operating Experience Quarterly, for the purpose of
gaining feedback from task team members. The publication consists of three parts, the
Executive Summary, Trends, and a List of Resources.
In addition, the Task Team has written a paragraph for inclusion in the guidance document
as to what types of events should be looked at to create good lessons learned. Lastly, the
task team is working on a letter to be sent to DOE-HQ/EH-33 regarding the re-engineering
efforts.
Policy Task Team - The Policy Task Team is working on an
"Interpretation Document" for DOE Order 232.1 and received many comments from OR
SIG members during the "Interpretation" sessions held during the Spring Meeting.
The task team will be promoting consistency in reporting through the use of the
"Interpretation Document" and will also be looking at different sites'
procedures for inconsistencies. The task team is also assisting in drafting the proposed
page changes for DOE Order 232.1 and will be discussing SSC equipment (consistency).
In addition, the task team will be looking at severity levels for occurrence reporting,
particularly off-normals. The task team is to develop recommendations on severity levels
(listing the pros and cons) and submit to EH. Fred Witmer, DOE/DP, has the lead on
drafting these recommendations.
Lastly, the Policy Task Team will be reviewing the discrepancies between DOE Order 151.1
and DOE Order 232.1. Jeannie Boyle and Earl Hughes, EH-33, are the DOE Points-Of-Contact.
Training Task Team - The Training Task Team has finalized the
update of the course materials on Introduction to Occurrence Reporting, Occurrence
Report Writing, and Reportable Occurrence Investigative Techniques to the new DOE
Order 232.1
In addition to the CBT course previously mentioned, they will begin working on a process
for certifying ORPS competency and will be determining which courses should be developed
and the type of delivery for proposed courses.
Trending Task Team - The Trending Task Team has four sub-task
teams which are working on the primer that the group plans to produce. The primer will
include examples, techniques, search strings and profiles, as well as, examples of
reports. The information in the primer will be ORPS specific. The primer will be issued as
a SIG product rather than a TRADE product but will receive an overall review by TRADE
before issuance.
The task team is also looking at whether additional fields are needed on ORPS for
analytical purposes. One of the Performance-Based Management (PBM) SIGs' strategic
objectives is to work with other SIGs to develop performance measures and Cynthia Eubanks
(who is a member of both the OR SIG and PBM SIG Steering Committees) will ask the PBM SIG
to assist the Trending Task Team in developing performance measures for trending.
Users' Group Task Team - The Users' Group reaffirmed their role
in the OR SIG as the task team responsible for providing input into enhancements to ORPS;
evaluating proposed enhancements and providing recommendations for or against
implementation; and beta testing all proposed software changes and any new software
relating to ORPS and the use of ORPS data. The Users' Group has pulled together input
received from various contractor and DOE sources on the development of a standard set of
keywords for the DOE complex to use in occurrence reporting. This information will be
consolidated into a list of keywords that will be made available on the ORPS Home Page and
on the ORPS Host computer. Responsibility for the keywords will be turned over to the
Trending and Analysis Task Team.
The Users' Group Task Team will be heavily involved in supporting the re-engineering
effort for occurrence reporting over the next year.
NEXT MEETING
The OR SIG Steering Committee and Task Team Leads will be meeting June 24-28 to finalize
preparations for the OR SIG Meetings to be held during the 1996 TRADE Conference. If you
